Team Travel Permit 

Application FORM for additional insurance for team special events:  The form is easy to complete.  It could take up to a week to get the approval. It is usually free of charge as long as the event follows certain conditions (such as wearing full gear, etc).  If you are unsure about which events are covered, consult:   Which events are sanctioned?
